Abstract

Introduction: Asthma is the most prevalent chronic disease among children, and this prevalence is still rising across many countries worldwide. In the Netherlands the prevalence of asthma among children is approximately 5%. Complications during pregnancy have been associated with childhood asthma. Maternal stress during pregnancy also seems to be an important risk factor. This study aims to investigate whether the association between maternal stress during delivery and childhood asthma at 45 months is mediated by complications during pregnancy. Maternal stress during delivery is defined based on the way the delivery is experienced by the mother. Methods: Data was used from the GECKO birth cohort. This is a population-based birth cohort. 686 children were included for analysis. Data was collected by questionnaires which were completed by the participating parents. Asthma medication use in the last 12 months was used as definition for childhood asthma at 45 months. A logistic regression model was used to analyze the associations described in the research questions. Associations were analyzed crude and adjusted for maternal smoking during pregnancy, birth weight, age of mother during delivery, parity, and pre pregnancy weight status of the mother Results: A ‘smooth/quick’ delivery increased the risk for childhood asthma (OR=2.17; 95% CI=1.16; 4.08). Also gestational diabetes (OR=5.62; 95% CI=1.33; 23.80) and pre-eclampsia (OR=4.99; 95% CI=1.34; 18.63) increased the risk of childhood asthma. The association between a ‘smooth/quick’ delivery and childhood asthma at 45 months was not mediated by complications during pregnancy (OR=1.86; 95%CI=0.95; 3.63). Conclusion: A mediation of the association between the way the delivery is experienced by the mother and childhood asthma at 45 months was not found. An association between gestational diabetes and pre-eclampsia as complications and childhood asthma at 45 months was. This study is the first in which stress during delivery and the possible association with childhood asthma is investigated. Future research has to focus on objective stress measurements during delivery, like cortisol measurements.
