Abstract
Purpose The goal of this randomized clinical trial was to compare the performance of experimental concave abutments with conventional straight implant abutments for solitary implants in the aesthetic zone. The primary outcome measure was the papilla volume mesially and distally of the implant. Materials & Method The study population consists of 26 patients with two missing contralateral teeth in the aesthetic zone in the same jaw. The teeth have been replaced by dental implants. One was restored with an experimental concave abutment (test group), the other with a conventional straight abutment (control group). Left and right were randomly assigned. A year after placement of the definitive crown, the papilla volume was measured distally and mesially of the implant by means of the papilla index scores (PIS). Dental radiographs have been made to determine the peri-implantary marginal bone level and the maximum bone level between the implant and the neighboring teeth. The effect of these two covariates has been statistically analyzed. Results The statistical analysis showed no significant difference in PIS between the test group and the control group (P=0,51). When regarding both covariates, there was still no significant difference found between the experimental group and the control group (P=0,346). Regression analysis showed a positive correlation between the maximum bone level and the papilla volume. It also showed a negative correlation between the peri-implantary marginal bone level and the papilla volume. Conclusion This study shows that for solitary implants in the aesthetic zone, a concave abutment design does not provide a better papilla fill than a conventional straight abutment.
