Abstract

Background: Trauma registry plays an important role in a process of improvement of health-service. The organization of trauma care in the Caribbean is limited; there is lack of a functional registry and distribution of trauma patients. Our objective was to compare incidence-trends of patients with trauma-related injury to the Dutch trauma registry for 14 years, since Curacao is part of the Dutch Caribbean. Methods: Data of all patients with single and multiple trauma-related injury (n=92,394) admitted to our Emergency Department (ED) from January 1, 2000 to December 31, 2013, were retrospectively analyzed using a prospective trauma registry. Results: In ED were 92,394 patients with trauma-related injury identified, 14,886 were selected: 9,390M and 5,496F. All were classified in groups: 10,392 traffic; 3,396 molestation; 574 firearm and 518 stab wound. In traffic-related injury were more men (p=0.025) than women, as well in the violence-related groups (p=<0.001). The incidence per 100,000 inhabitants per year of traffic-related trauma presented in the ED was 529.7 (95% confidence interval [CI]:430.9-628.5), firearm-related 29.2 (95%CI:18.3-40.2), stab wound-related 26.4 (95%CI:18.4-34.4) and molestation-related 173 (95%CI:127.5-218.5). There is an overall decline in incidence over 14 years. There is no difference in incidence per month. At the end of the month after payday (p=0.005) and in the weekend (p<0.001) are significant more traffic-related injury. On Saturdays there are more stab wound-(p=0.036) and molestation-related trauma (p=0.001). Hospitalization of traffic-related injury was 22.7%, firearm-related 48.3%, stab wound-related 30.5% and molestation-related 18.9%. Hospitalization rates are stable over the years. All trauma-related injury were highest among people aged 15-34 years (p<0.001). Conclusion: Hospitalizations rates differed per trauma-related injury, as well as the follow-up. The incidence and trauma mechanism seem age- and gender-related. Trauma occurs most of the time in the weekend and traffic-related trauma after payday. Despite the overall decline in trauma incidence, hospitalization rates are the same. Comparing Curacao with The Netherlands, there is a higher incidence of trauma-related injury, but comparable with countries in the region and the United States. The current trauma registry is too limited to score international criteria. Curacao’s registry will be linked to the Dutch ‘Datadictionary National Database’. This study is a start to gather data of traumarelated injuries to improve the health-system of Curacao.
