Introduction: The aim of this research is to determine if there is a decrease in hemoglobin (Hb) in the acute phase of sepsis. During sepsis degradation of the glycocalix occurs, a carbohydraat-rich layer of 0,2 – 0,5 µm covering the luminal side of the capillaries, resulting in an increase of the intravascular space and thus decrease of Hb. Patients and Methods: In this retrospective study on the Intensive Care Unit (ICU) of the Medical Centre Leeuwarden we used a database which concludes the clinical data of patients admitted in the last decade. Patients who fulfilled the international criteria of sepsis with minimal one sign of organ failure during ICU admission, were included in the sepsis group (S group, n =296). The control group was formed by patients admitted urgently to the ICU (C group, n = 320). Serum laboratory values , determined in blood taken at first presentation in the hospital, were collected and tested for significant differences between groups and correlations between primary and secondary outcome variables. Results: The difference in Hb between the S and C group was not significant (8,76 ± 1,18 mmol/l versus 8,93 ± 1,16 mmol/l, p=.07). After correcting for a number of confounders using a multivariate regression analysis, we observed a significant difference in Hb of -0,23 mmol/l in the S group at relative to the C group (p=.01). Conclusion: During first presentation in the hospital there is, after correction for confounders, a significantly lower Hb in patients with sepsis at relative to a control group, without the existence of anemia.
