Abstract

Background and purpose: The Control of Allergic Rhinitis and Asthma test (CARAT) was created with the purpose to get information about asthma control and allergic rhinitis control. It consists of an asthma control sub score and a rhinitis control sub score. In this study the effect on giving a diagnosis and treatment advice using either the CARAT or the standard questionnaire used by the Asthma and COPD-service (Certe A/C-service) in the Netherlands, the Asthma Control Questionnaire (ACQ), are compared. The CARAT asthma scale, is expected to be as useful as the ACQ. In addition, the added value of the allergic rhinitis questions in the CARAT are evaluated as well as the user friendliness of the CARAT. Methods: In this experimental pilot study, 100 randomly chosen patients were included and their asthma control and rhinitis control was, amongst other things, evaluated by 11 different pulmonologists. The data that was used came from the Certe A/C- service. The pulmonologists were asked to give a diagnosis and a treatment advice for each patient using two evaluation questionnaires, one including the CARAT and one including the ACQ. A Pearson’s Chi Squared test was used to determine the differences in treatment advices between the ACQ and the CARAT and also between the group patients suffering from asthma with allergic rhinitis and the patients suffering from asthma without allergic rhinitis. The user friendliness of the CARAT was tested by a self created usability questionnaire filled in by the experts. Results: A significant difference (p < 0,000) was found between the ACQ and the CARAT questionnaire regarding a treatment advice focused on allergic rhinitis. For the 20 other treatment advices, no differences were found between the ACQ and CARAT. Another outcome of this study is that there is a significant difference (p < 0,018) in the combined medication treatment advices between the group asthmatic patients with allergic rhinitis and without allergic rhinitis. There were no differences found between these two groups in the other treatment advices. The majority of the experts (81.8%) considered the CARAT to be as useful as the ACQ and most of the experts (80,0%) would recommend to add the CARAT to the Certe A/C-service. Conclusion: It can be concluded that the four allergic rhinitis questions used in the CARAT lead to a difference in one of the treatment advices given by the experts. The majority of these experts think that the CARAT is as useful as the ACQ and should play a role in the Certe A/C-service. Therefore, it has to be considered, that the CARAT will be added to the Certe A/C- service in the future.
