Abstract

Introduction In future years, the incidence of cancer patients will increase due to the aging and growing world population. This results in a greater number of elderly patients undergoing surgical procedures. Postoperative complications, such as postoperative cognitive dysfunction (POCD), are more common in elderly patients. This study examines the influence of intraoperative hypotension on the development of POCD in elderly cancer patients. Methods Data for this study was derived from the PICNIC B-HAPPY study, a prospective observational cohort study. The patients included in this study are at least 65 years of age and underwent surgery for a malignant tumor. POCD is defined as a deterioration of one standard deviation in at least one test in the postoperative test results after three months compared to the preoperative test results. During the surgery, absolute hypotension, relative hypotension and the severity of hypotension were measured. Data was analyzed using Mann-Whitney U test, independent t-test, univariate logistic regression and multivariate logistic regression. Results 53 patients were included in this study. Of these, 12 developed POCD after three months (23%). Absolute hypotension (MAP 50-59 or < 50 mmHg, p = 0,261) or relative hypotension (30-39% or > 40% decrease, p = 0,175) were not significantly associated with POCD. Furthermore, the severity of intraoperative hypotension (depth * duration) did not influence the development of POCD. Conclusion Independent of the applied definition, intraoperative hypotension was not associated with the occurrence of POCD in elderly cancer patients.
