Abstract

Background. A neck dissection can cause neck and shoulder pain and disability. There is no clear data on neck and shoulder disability following a neck dissection for thyroid cancer. Thyroid cancer patients will suffer on the very long term of neck and shoulder disability, because of their high survival rate. Methods. VAS neck and shoulder pain scores, SPADI, NDII and range of motion of the neck and shoulders were obtained from 103 thyroid cancer patients in two groups. Fifty-eight patients underwent a neck dissection and 45 underwent a thyroidectomy only. Results. VAS neck pain score, SPADI pain score and NDII differed significantly between neck dissection and thyroidectomy patients. Ranges of motion did not differ significantly. Cumulative radioiodine dose significantly related to abduction and forward flexion of the shoulder. Conclusions. Neck dissection patients perceive more neck pain compared to thyroidectomy patients. They do generally not perceive more shoulder pain (except under increased demands of the shoulder) nor more limitations compared to thyroidectomy patients. Yet, they do perceive greater neck and shoulder disability -and thus, experience lower quality of life- compared to thyroidectomy patients.
