Abstract

Introduction Colorectal cancer is the second most common cancer in the Netherlands. Colorectal screening was introduced in the Netherlands in 2014. Screening test have -beside benefits- some disadvantages, including possible psychosocial distress following a false positive colorectal screening test. In the case of colorectal carcinoma, limited information is known about the prevalence of psychological distress following a false positive screening test. Even less is known which patient characteristics predict psychosocial distress. The aim of this study is to examine if patients develop psychosocial distress 6 weeks after the coloscopie and which patient factors are related with psychosocial distress after a false positive test. Method This cohort study used data from a larger, ongoing study (OnCohort) into psychosocial distress. The primary outcome is psychosocial stress, assessed through the Psychological Consequences of Screening Questionnaire (PCQ), the secondary outcome is cancer worry, assessed through the Cancer Worry Scale (CWS). Patient characteristics were obtained by questionnaires. With univariate and multivariate logistic regression analyses was examined which factors were associated with psychological distress. Results In this study 80 participants were included. The analyzes showed that 50% of the participants had no stress, 75% had little stress. In 80% of the participants, there was no to few cancer worry. In univariate analyzes, a lower age, more emotional instability and more hypochondria appear to be significantly associated with psychosocial stress. The best-fit model for predicting psychosocial stress includes age and emotional instability (age OR: 0,88 95%CI: 0,78–0,98; emotional instability OR: 1,37 95%CI: 1,12–1,67; Nagelkerke R2: 0,29). In analyzes of cancer worry, emotional instability appears to be significantly related(OR: 1,25 95%CI: 1,04-1,65). Conclusion Most of the participants had no to little stress 6 weeks after the coloscopie, 80% had no to little cancer specific concerns. In the analyzes, a younger age and more emotional instability appear to be related to the development of psychosocial stress. A general practitioner can be more alert with younger patients and patients who are more emotionally instable at the development of psychosocial stress after a false positive screening test and offer extra psychological help.
