Abstract

Background: Suicide is the leading cause of premature death in patients with psychotic disorders, with high rates in the first years of the disease. Individuals from the province of Groningen that seek help for a first episode psychosis (FEP) are referred to the University Centre for Psychiatry in Groningen for the ‘Early Psychosis Outcome’ protocol (EPO). Early intervention in psychotic disorders could prevent reoccurrence of psychosis. Early intervention may also decrease suicide risk. Aims: To determine the suicide risk after first episode psychosis and investigate the possible predictors for suicide. Methods: A retrospective observational study of patient files of 614 subjects who completed the EPO between 2000 and 2009. With file search we determined whether a subject had committed suicide, or else if he/she was still in care, moved out of the three northern provinces, or had deceased because of other causes than suicide. Suicide risk and predictors for suicide were established using Cox regression, with censoring for the subjects who are lost to follow up because of the aforementioned reasons. Results: Of 190 subjects the current status was unknown and were not included in the Cox regression. 9 subjects were deceased because of suicide, 7 because of a different cause. 17 subjects moved and 31 subjects left the psychiatric care. Of the 9 suicides, 6 took place within 1.5 years after referral for FEP. Higher age during assessment was a significant independent risk factor of suicide. With every year of age, there was a 1.1% increase on the odds of the risk of suicide. Conclusions: 2.1% of subjects died of suicide after referral for FEP. Most suicide completions occurred in the first year and a half after referral for FEP. In addition, higher age during the assessment was associated with a higher risk on suicide. Continuation of this study is necessary to get a more complete representation of suicide after referral for FEP.
